Personal Health Summary

The Personal Health Summary is a function that provides a way for you to print out some, or all of the health history information that you have recorded on My HealtheVet. This handy print-out may help you communicate your health information to your doctor and his records system.

Using this Page

Select Categories - Select which categories of information you wish to display on your printable summary by checking the boxes next to each item.

Create Doctor Sheet - By clicking this button, My HealtheVet will generate a printable "Doctor's Sheet" containing the information you selected above.
